About

Your local Care Navigator offers free impartial advice aimed at people over 50 who feel they need extra support to remain independent in their own homes.

This service can offer help and information to guide you through a variety of issues and generally improve your sense of well-being by:
• Providing awareness of benefits
• Providing awareness of community services
• Developing a Support Plan if you require support at home
• Assessing & supplying equipment and minor adaptations to your home
• Assist with some form filling ie Blue Badge applications
• Assist with planned hospital admissions and hospital discharges
• Helping you take charge of and manage your own affairs

The service is confidential, impartial and non-judgemental and is designed to make life easier for older people when dealing with problems or changes in their lives.
